Acknowledgements
Platform Usage Acknowledgement:
The program/course/service will use a combination of Slack, Zoom, Google, and other platforms to host our meetings and other announcement. By checking the box from the link next to it, to which this page is connected, I agree to comply with the data policies of these platforms and use them as directed by Harmony Plus instructors.

PROGRAM Role Acknowledgement:
By checking this box, I signify that I understand the purpose of program/course/service is to cultivate student skills and increase competitiveness of student resumes. I understand that Harmony Plus does not guarantee internship or volunteer placement by participating in this program/course/service.
Waiver
As a material inducement for Harmony Plus Inc., a corporation duly organized and existing under the laws of the State of California (“Harmony Plus”) to accept participation by (STUDENT NAME), (hereinafter “Student”) in the program/course/service, for good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, If student is a minor, I, (PARENT NAME) as parent or legal guardian of Student, hereby agree as follows.
The undersigned affirm that Student’s participation in the program/course/service is entirely voluntary. Student agrees to learn and obey, and the parent or legal guardian of Student has instructed Student to obey, all the rules, regulations and instructions of the Student staff, including without limitation, that Student shall not leave the Student at any time without permission of an authorized program/course/service staff and that Student must attend classes on time, pay attention in class, follow instructions, and be respectful to faculty, staff, and fellow students.
In consideration for permitting Student to participate in the program/course/service, the undersigned voluntarily agree, for themselves, their heirs, executors, and administrators, to release, waive, hold harmless, indemnify, forever discharge and agree not to sue Harmony Plus, the program/course/service, or their respective officers, directors, shareholders, trustees, employees, agents, students, and staff from any and all liability, claims, actions, demands, expenses, attorneys fees, breach of contract actions, breach of statutory duty, or other duty of care, warranty, strict liability actions, and causes of action whatsoever, that the undersigned might now have or may acquire in the future, arising out of or in connection with the Student’s participation in the program/course/service.
The undersigned acknowledge and agree that they shall not publicize or distribute any information or any materials furnished by Harmony Plus or the program/course/service, and shall not use such information and materials for any purpose other than participating in the program/course/service. Additionally, the undersigned agree that the Harmony Plus and the program/course/service (including their respective staff, employees and agents) can, to the maximum extent permitted by the applicable law, take and use photos, videos and audio recordings of the undersigned and the undersigned’s feedback concerning the program/course/service for marketing, public relations, and other lawful purposes.
The undersigned hereby give consent for emergency medical care prescribed by a licensed doctor of medicine. The undersigned further understand that they shall be responsible for Student’s medical expenses.
The undersigned acknowledge that they have been advised of, and are familiar with, the provisions of California Civil Code Section 1542, which provides “[A] general release does not extend to claims that the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release and that, if known by him or her, would have materially affected his or her settlement with the debtor or released party.” Being aware of said code section, the undersigned agree that the release of claims set forth herein is a general release, and that they expressly waive any rights they may have under California Civil Code Section 1542, as well as under any other statute or common law principles of similar effect.
In the event that any clause or provision of this Release and Waiver of Liability and Parental Consent shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release and Waiver of Liability and Parental Consent which shall continue to be enforceable.
By signing this portion of the form, I acknowledge that I have read, understand and agree to the terms outlined above:
